Every motor carrier must maintain a driver qualification file for each driver it employs. You will be sent a Driver Application for Employment with instructions for filling out the form. After completing the form, you send it back to our company. Then we will exchange the information with you and build a DOT Compliant Driver Qualification File to include your driving record which is required for your audit. If you live in Utah, we are unable to obtain your driving record.
